A new Finnish study has sharpened what we know about coffee and body fat: among 2,264 adults who were all exactly 46 years old, the heavier coffee drinkers carried less fat and more muscle than the lighter ones — even when their body mass index was identical. The number on the weighing scale said they were the same. Inside, they weren't.

The work comes from the Northern Finland Birth Cohort 1966, a group of people tracked since birth, and was published in the European Journal of Nutrition in July. Medical Xpress and News-Medical reported the findings this month. The gaps showed up in three places: total body fat, visceral fat — the deep kind packed around the organs — and skeletal muscle mass.

Then came the part nobody was looking for. The researchers also measured sex hormones, and the pattern split cleanly along sex lines. That matters here more than it does in Helsinki, because India is carrying a metabolic load that Finland is not: ICMR-INDIAB, published in The Lancet Diabetes & Endocrinology, counted 101 million Indians with diabetes and 351 million with abdominal obesity.

Same BMI, two very different bodies

BMI is height and weight, nothing else. It cannot tell muscle from fat, and it cannot tell fat under the skin from fat wrapped around the liver and pancreas. That second kind is the one that tracks with insulin resistance and heart disease. So a study finding that two groups match on BMI but differ on visceral fat is not a small technicality — it is the whole point.

This lands hard for Indian readers. South Asians tend to carry more fat, and more of it centrally, at any given BMI than Europeans do. That is exactly why Indian guidelines treat 23 as the overweight threshold rather than the Western 25. If a 46-year-old Finn's scale can hide a fat difference, an Indian one can hide more.

The hormone split between men and women

The clearest signals were in men, who also showed a more favourable glucose-insulin profile at higher coffee intake. Both sexes shared one finding: lower levels of circulating branched-chain amino acids, which are flagged in the research as markers that rise with insulin resistance and diabetes risk when they stay chronically elevated.

In men: more testosterone, less of it free

Higher coffee intake went with higher total and bioavailable testosterone, and higher sex hormone-binding globulin — the protein that ferries testosterone around and keeps much of it locked up. Because SHBG rose, free testosterone and the free androgen index came out modestly lower. So the headline "more testosterone" is only half the sentence, and the half that gets dropped changes the meaning.

In women: mostly a shift in SHBG

Women showed higher SHBG too, and lower free androgen measures, but little else moved. The researchers stress this is an observational study across one age group in one country. It shows associations, not proof that coffee caused any of it.

What stood out in our findings was a distinct hormonal signature that didn't disappear even after we took into account BMI and lifestyle factors, said lead author Luca Verroest, a doctoral researcher at the University of Oulu.

Where coffee and body fat fit with older research

This isn't a lone result. In 2023, BMJ Medicine published a Mendelian randomisation study using variants in the CYP1A2 and AHR genes, which govern how fast the body clears caffeine. People genetically wired to hold higher blood caffeine levels tended to have lower BMI and lower type 2 diabetes risk, with roughly half of the diabetes effect running through body weight.

Genetic studies like that one dodge the biggest weakness of surveys — that healthier, wealthier, less sedentary people may simply drink more coffee. Read together, the two point the same way. Neither proves that adding cups fixes anything, and coffee is hundreds of compounds, not just caffeine.

Why India's cup is not Finland's cup

Finland routinely tops global per-capita coffee tables, and the local habit is black filter coffee, usually unsweetened. India is at the other end. Per-capita consumption sits near 0.04 kg a year against a world average around 1.3 kg, though domestic demand has been climbing 7-10% annually — roughly 95,000 tonnes in 2024, up from 80,000 in 2020.

The catch is what goes in the cup. A South Indian filter coffee is mostly milk with sugar; a café latte at ₹250-plus can carry flavoured syrup on top of that. None of the Finnish findings survive if the drink itself adds 150-200 calories of sugar and full-fat milk. The study looked at coffee, not at coffee-flavoured desserts.

What to do with your next cup

Nothing here justifies a fifth cup, and nobody in this research told anyone to start drinking coffee. But if you already do, a few things are worth checking:

  • Measure your waist, not just your weight — visceral fat is what the scale misses.
  • Count the sugar. Two teaspoons per cup, four cups a day, is a habit worth auditing.
  • Watch the timing if you sleep badly; poor sleep undoes metabolic gains faster than coffee builds them.
  • Talk to a doctor before changing intake if you have hypertension, arrhythmia, acid reflux or are pregnant.

What to watch next: whether the coffee and body fat pattern holds in trials that actually assign people to drink coffee, and whether it repeats in South Asian populations, where the underlying body composition is different to start with. Until then, treat this as one more reason to take your coffee less sweet — and to stop trusting the number under your feet.
