A Harvard-led analysis of more than 12 million death records found cabin crew and pilots lead every other occupation in the share of deaths from radiation-linked cancers — and almost nobody is measuring their dose.

A new Harvard-led study puts the flight attendant cancer risk ahead of every other occupation it examined — more than 500 of them — when you measure the share of workers who die from cancers tied to radiation. Pilots landed second. Both groups finished ahead of people who handle radioactive material for a living.

The paper appeared Aug. 18 in JAMA Internal Medicine and leaned on more than 12 million U.S. death certificates that recorded the person's job. Cancers linked to radiation made up 6.9 per cent of flight attendant deaths and 6.7 per cent of pilot deaths. Across the working population as a whole, the figure sat near 5 per cent — roughly one in 14 compared with one in 20.

Put another way, cabin crew had about 50 per cent higher odds of dying from one of these cancers, and pilots about 36 per cent. The suspected culprit isn't anything on board the aircraft. It's cosmic radiation from space, which mostly gets filtered out before it reaches the ground but arrives at close to full strength eleven kilometres up — and it climbs the nearer you fly to the poles. Canadian routes do plenty of that.

How Two Airline Jobs Outranked Nuclear Workers

This ranking measures proportion, not head count. Flight attendants are not dying of cancer in enormous absolute numbers; rather, among those who do die, an unusually large slice is a radiation-linked type. Measures like that can mislead when a group happens to dodge other causes of death. So the researchers ran the obvious check, and neither pilots nor cabin crew showed elevated deaths from cancers that have nothing to do with radiation.

Where the Flight Attendant Cancer Risk Shows Up

The cancers counted as radiation-linked were leukemia, lymphoma, multiple myeloma, and cancers of the skin, breast, thyroid, prostate and central nervous system. For both jobs the odds ran high across every one of those categories instead of spiking in a single outlier. In a commentary published alongside the paper, Olsen and Karipidis argued that this spread points at exposure on the job rather than lifestyle or income as the explanation.

Why Canada's Cosmic Radiation Rules Are Only Advice

Canada is not silent on this. Transport Canada issued an advisory circular on April 5, 2001, asking air operators to build programs for managing crew exposure. Anyone likely to absorb more than 1 millisievert a year should be treated as occupationally exposed, it says, with doses kept as low as reasonably achievable. The recommended ceiling is 20 mSv annually, which working crew rarely come close to — typical estimates land between 0.2 and 5 mSv.

The catch sits in the word advisory. It's a recommendation to airlines, not a rule with an inspector behind it, and it turns 25 next year. Cosmic radiation also falls outside the Canadian Nuclear Safety Commission's mandate because it isn't man-made. The United States is looser still: the FAA concedes the exposure happens but sets no dose limit and mandates no monitoring.

"You can't manage an exposure you've decided not to measure," said Dr. Vishal Patel of Harvard Medical School, who led the study.

Sara Nelson, president of the Association of Flight Attendants-CWA, made a companion point in coverage of the findings: the hazard is known, yet crew are not educated or informed about it. There's a practical cost to that gap. A crew member with no personal dose record who develops one of these cancers 30 years later has little documentation to bring to a workers' compensation claim.

The study's authors suggested concrete screening steps rather than career changes. Their recommendations point toward earlier and more regular checks for the cancers that showed up most:

  • An annual skin examination, given how strongly melanoma and other skin cancers featured
  • A conversation about starting mammograms earlier, or having them more often, for women flying regularly
  • Keeping your own log of flight hours, routes and years of service, since no one else is keeping a dose record for you

If you're a frequent flyer

Nothing here should change how you book a trip to Vancouver or Delhi. The flight attendant cancer risk described in this research reflects a working lifetime aloft — thousands of hours a year, decade after decade. Someone taking 20 flights a year absorbs a small fraction of that. Business travellers who log 200-plus flight hours annually sit somewhere in between, and would be reasonable to mention it at a physical.

What to raise with your doctor

Occupation rarely comes up in a routine appointment. If you spent years flying, say so, and say how long. The study's blunt takeaway for physicians was to treat a career at altitude as grounds for a slightly lower threshold of suspicion — worth a mole check, worth taking odd fatigue or persistent swelling seriously rather than waiting.

What to Watch For Next

Two things are worth following. First, whether Transport Canada modernizes that 2001 advisory into something operators must actually do, which is the change unions on both sides of the border have pushed for. Second, whether anyone runs the equivalent analysis on Canadian mortality data — this study used U.S. records, and no comparable Canadian breakdown of the flight attendant cancer risk exists publicly. Neither will move fast. In the meantime, if you fly for a living, ask your employer what dose estimate it uses for your routes, book the skin check, and start writing down your hours.
